HOW DO I COMPLETE MY “E-VISIT”?
  1. Take three pictures of your face.

  2. Have available all of the current medications that you are taking by mouth or applying to your skin, so you can list them in your “E-Visit”.
  3. Click on the RelayHealth logo (below) to complete your acne visit on-line. Follow the steps listed below on this page.

Brief Instructions for Relay Health Website:
  1. Take 3 photos and save them to your computer.
  2. Sign in to “relayhealth.com” with your username and password
  3. Choose “Consult Your Doctor” on left side of screen
  4. Select a doctor and click “Next”
  5. “Consult your doctor about current health problem”, and click “Next”
  6. Update your Insurance Information. Click “Yes” to proceed and “No” to update.
  7. Follow prompts and click on “ I Accept” to agree to terms
  8. Under “Choose a Symptom” click on “Adult Acne”
  9. Complete visit Interview by answering all the questions.
  10. The next section is “Update your Health Record”. Here you will add your photos.
    • Click on “Add/Delete Files” on the right side of the screen.
    • Click on the Browse button to locate your photos.
    • Click on “Add Attachment” to attach the photo.
    • Repeat these steps to attach each photo
    • Once you are finished adding all 3 photos, click on “Done”
  11. You are now back in the “Update your Health Record” screen
  12. Update your health records as necessary and press “Next”
  13. Enter your co-pay information
  14. Click on “Send” to send the visit to your doctor
  15. Your visit is now complete and your doctor will reply within 3 business days.
